Reporting a Lost Animal Form

To report a lost animal, complete the form below.  Please call Animal Services at (630) 407-2800 x0 with any questions.

If your animal has been lost for more than 1 day, please complete this report and also call the above number to have a staff member check against all stray animals in our database.   Animal Services cares for nearly 2,500 animals each year, so please provide as much detailed  information as possible to assist our staff in matching your lost report to any incoming stray animals.  After you have completed your lost report, please email a copy of your pet's Lost flyer to animalservices@dupageco.org.  We also recommend coming into the shelter during general business hours to view our stray animals.
