Job Description

DEFINITION:
Under general supervision; performs work of moderate difficulty in providing professional social work assistance to patients and/or clients; performs related work as required.

DISTINGUISHING FEATURES OF THE CLASS:
An employee in this class provides professional social service assistance to patients and/or clients by coordinating treatment plans, providing references to community support networks and services, acting as an advocate, developing programs and arranging for other assistance. General supervision is received from the assigned department head or supervisor, depending on area of assignment.

EXAMPLES OF DUTIES TO BE PERFORMED WITH OR WITHOUT REASONABLE ACCOMMODATIONS:
Provides patients and/or clients and their family members with agency and resource information regarding community services; coordinates long-term planning process for clients; provides input in developing intervention strategies; explains processes and discusses needs; records information for tracking new patients and/or clients; interacts with members of the interdisciplinary team, community resources and schools; develops programs; records personal observations regarding progress; completes progress notes and prepares annual report on unit activities; utilizes the computer to communicate information; attends meetings; completes follow-up with patient and/or client families in a timely manner. Adjusts work assignments and schedules in the event of an emergency to participate in emergency preparedness, response, and recovery activities as assigned; depending on assignment may maintain required emergency management training, licensure and/or certifications.

DESIRABLE KNOWLEDGE AND SKILLS:
Considerable knowledge of the availability of community healthcare and social service resources; considerable knowledge of professional standards, practices and techniques in the field of social work; considerable knowledge of medical and psychological terminology, behavior patterns and illness effects.

Considerable skill in understanding problems, needs and motivations of patients and/or clients and families; considerable skill in managing multiple priorities; working skill in dealing effectively with others; working skill in using language effectively; working skill in exercising sound judgment in keeping with current standards and practices of the social work profession.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:
Employee may be required to stand, walk and occasionally reach with hands and/or arms to provide care, crouch or stoop, smell, lift and transfer up to 150 lbs, depending on area of assignment.

SUGGESTED TRAINING AND EXPERIENCE:
Completion of a Bachelor’s degree in Social Work or a related field and two years of experience as a social worker or an equivalent combination of training and experience.
