Awarded CDC Project contractors are required to send bid notification to any appropriate MBE/WBE companies on our list. If you would like your company to be placed on our MBE/WBE list, please complete and send the Recognition Application us along with the requested back-up documentation.

Certification Affidavit

(Please Note: If your company can provide proof of MBE/WBE certification with the City of Chicago, the Illinois Department of Central Management or by the Chicago Minority Business Development Council, Inc., DuPage County will accept this certification and you will not need to submit additional forms.)
