All forms may be completed online and then print out the form, or you may  print the form and then type in the necessary information or neatly print the information in black ink in the spaces provided.

If you are an attorney and wish to file any paperwork in the 18th Judicial Circuit Court, you must have a DuPage County attorney number.  If you do not have an attorney number, or if your information has changed since you have received your attorney number, please complete the Attorney Information Sheet to receive a number or to update your information.

It is your responsibility to fill out these forms completely. Legal information is available at the Law Library.
